Begin by blending the fresh blueberries with 1/2 cup of the granulated sugar and the lemon juice until smooth. Strain the mixture through a fine mesh strainer to remove any pulp, leaving a smooth puree.
In a saucepan over medium heat, combine the remaining 1/2 cup of sugar, heavy cream, and whole milk. Stir continuously until the sugar is dissolved and the mixture is warm to the touch, about 5 minutes. Do not let it boil.
Remove the saucepan from heat and whisk in the crème fraiche and vanilla extract until fully incorporated, forming a creamy base.
Combine the blueberry puree with the cream mixture, stirring until well mixed.
Chill the mixture in the refrigerator for at least 2 hours or until thoroughly cold.
Pour the chilled mixture into your ice cream maker and churn according to the manufacturer's instructions. It should reach a soft-serve consistency in about 20-25 minutes.
Transfer the ice cream to a lidded container and freeze for at least 2 hours, or until firm, before serving.
